The short version, before the detail. The cheapest ordinary code for the base game is on PC, the new story content is a second purchase on top of it, and on console neither of those holds true in the same way. What the sellers have for each platform is very different, and on PlayStation they have almost nothing. Here is where the money actually goes.
Cost of Hope is sold separately, at its own price
The expansion has not been folded into the base game. It is its own product, with its own sellers and its own spread, and the cheapest ordinary code for it is € (with coupon SEPGOC10) at Kinguin. The base game is a purchase of its own.
That distinction matters if you are coming back after a long break. The patch that landed alongside the expansion is free and needs nothing bought: it arrives through the launcher. The story content does not. If the base game is already in your library, the expansion is all you need to look at. If you are starting from zero, you are paying twice.
The PC route: two purchases, one launcher

What each platform asks for the same Zone
The console versions are catalogued under the short name and the PC one under the full title, but it is the same game. Side by side, at whatever the sellers are asking right now:

Two things stand out. Nobody is selling the expansion for console here, so a console player who wants the story content is buying it from the platform's own store at whatever that store asks. And the PC base game is the cheapest ordinary code of the three, which is not what most people assume about a game this demanding.
On PlayStation there is nothing here for you
Worth saying bluntly, because it changes the decision. Every PS5 offer on sale right now is an account offer. No code, no gift, nothing you redeem on your own profile. The cheapest of them asks € (with coupon Gocdkeys), which is more than an ordinary code costs on either of the other two platforms.

Do I need the base game to play the expansion?
Yes. Cost of Hope is story content built onto the base game, and its code is worthless on its own. Starting from nothing means budgeting for both.
Does an old save still work?
Saves from earlier versions usually survive the big patches. A heavily modded install is another matter: mods built against an older version are the standard reason a returning player's game refuses to start. Turn them all off, then bring them back one at a time.
Is the Xbox version the same game as the PC one?
It is. The console entry carries the short name and the PC entry the full one. That is how the stores named them, not two different products.
Where we would actually buy it
PC, in two steps: the base game first, taking an ordinary code rather than one of the accounts below it, then the expansion on top. The base game alone is the cheapest ordinary code of any platform here, and both halves end up in a library you control.
