Aooni: The Horror of Blueberry Onsen transports the legendary Ao Oni franchise to a new setting—an abandoned hot spring inn rumored to be haunted by supernatural forces. The once-popular resort, known for its beauty-enhancing waters, now stands in ruins as a backdrop for mystery and dread. Players join Hiroshi, Takeshi, Mika, and Takuro as they investigate the location while recording content, only to encounter a suspicious stranger and uncover the dark secrets of the Blueberry Hot Spring hidden within the inn's depths.

Gameplay centers on a cycle of exploration, puzzle-solving, and evasion. Players navigate the decrepit facility, uncovering clues and solving environmental puzzles to progress. The core tension comes from encounters with the Ao Oni—a blue humanoid creature that stalks the halls. When spotted, players must flee and hide in lockers or other safe spaces, waiting for the threat to pass before resuming exploration. This loop of discovery and survival forms the foundation of the experience, with multiple demon variants adding variety to encounters.

The game introduces several new systems to enhance replayability and accessibility. A high-speed mode allows players to adjust gameplay speed from 2x to 16x, enabling both intense challenges and expedited playthroughs. Online ranking functionality lets players compete based on completion scores. Cooperative play with friends and character conversations add social and narrative depth compared to earlier entries. Multiple endings branch based on player choices, encouraging repeated playthroughs to uncover the full mystery.

Aooni: The Horror of Blueberry Onsen appeals to fans of classic survival-horror puzzle games and those seeking a modernized take on the original freeware phenomenon. The title balances deliberate pacing with puzzle variety, offering clever environmental challenges alongside chase sequences. While the game maintains the retro aesthetic and straightforward mechanics of its predecessor, the new features and setting provide fresh content for both series veterans and newcomers to the Ao Oni legacy.
