Magical Girl ER is a horror-tinged medical simulation that flips the magical girl genre on its head by focusing on the aftermath of battle. You take on the role of a temporary emergency hospital director treating magical girls injured in kaiju-related disasters. Rather than celebrating heroic victories, you examine wounds, diagnose injuries, and make critical treatment decisions with limited resources.

The core gameplay revolves around triage and diagnosis. When patients arrive, you examine them using tools like thermometers and stethoscopes, gather clues from dialogue and visual details, and infer hidden conditions from the evidence before you. Each patient presents a puzzle: their symptoms may be misleading, their injuries complex, and your diagnosis could be wrong. Treatment options range from simple bandages to full surgery, and incorrect decisions carry consequences.

Resource management adds constant pressure to your choices. Hospital beds are limited, your stamina depletes, and you must decide which patients to accept and which to refuse. Every admission and treatment affects the hospital's reputation, patient survival rates, and the story's outcome. The game incentivizes multiple playthroughs through newspaper reports, visitor events, and multiple endings that shift based on your decisions throughout the game.

Developed by Japanese illustrator and VTuber noranuco, Magical Girl ER combines dark psychological horror with strategic decision-making. The game explores themes rarely addressed in magical girl media: the real cost of heroism, the weight of medical responsibility, and the moral ambiguity of triage. This title appeals to players seeking narrative depth, strategic resource management, and a fresh take on familiar genre tropes.
